Required Documents for Applicants:

*Background Screening Level 2 that is Eligible*

  • License/Certification (Nurse license, self-medication certificate, CPR certificate) 

  • Social Security Card or documents authorizing applicant to work in the U.S.

  • Driver's License/Valid Indentification

  • HIV Certificate

  • Physical - must be within the past 6 months, stating that applicant is "free of communicable disease" (cannot be performed by a Chiropractor) 

  • Professional Liability Insurance

  • No Disqualifying Criminal Record

  • Strong command of the English Language (bilingual is a plus)
