Both you and your referral will receive a $20 Amazon gift card once the referred Provider completes the registration process as a contractor with Care Alternatives and completes their first Care Alternatives patient assignment.*

*A patient assignment is considered complete once the health care provider completes 20 hours of paid services with a satisfactory review from the patient.
